Physical Exams - REQUIRED BY END OF JULY

• NISD requires every student in band and athletics to complete a physical.

• If you already have one with coaches or trainers, the Taft Band Staff must have a copy on file. Please ask them to email a copy. If we do not receive it, another one is required.

• All students must have a current questionnaire form on file each year. If the answer is “yes” to any of the questionnaire documents, a current physical must be on file.

• Physical exam is good for 2 years from the completion date. If you got a physical last summer, you are still covered through this season.

• Turn this in at your assigned “Money Night” day- not over the summer.

• Urgent Care Center at 1604 and Shaenfield is an easy location to do a physical.
