Required physical forms
Each year, all students must fill out the UIL Pre-Physical Questionnaire. If you answer "yes" to any questions on this document, a valid paper copy of a physical must be on file with Mr. Mills. The physical must have been done within the last 18 months to be valid.
UIL Physical Form (all students must have a valid physical on file. A physical is valid for 24 months from the exam)
2024-2025 Student Enrollment Forms
All forms are to be filled out PRIOR to Money Night. Please do not email these forms to directors, they will be delivered in person. Copies will be available if you do not have access to a printer for students at money night. Thank you!

Please view and reference the following forms for our 2023 Money Night. Please note that Color Guard students use a different form than Percussion and Winds do. We will have printed copies at money night, so no need to print.
